The links below entitled Registration Forms and Health History Form contain paperwork that will need to be filled out prior to your first appointment. Please print out the forms at work or at home, complete, and bring them to your first appointment. You can also submit them to our office via e-mail at office@lindblomdental.com.

If you elect to fill out the forms in our office, please arrive 15-20 minutes prior to your scheduled appointment to allow for enough time to complete your forms and begin your appointment promptly. Thank you and please contact our office if you have any questions.
